i dont have one yet but the full throttle hand brake looks like the way to go. its kinda spendy. if i had the money for one it would be on there.

do they need to be geared much for slow stuff?

The only reason you need to gear a bike down at all is for circles so yeah if you want to turn circles you gotta gear them down. I dont have a handbrake yet on my crf250 mostly because the rear caliper piston is so small a handbrake is not going to have much stopping power. Trying to figure out what to do with that now.
