double check both the aluminum nuts. i had one one time where there was a hairline crack in it and that was the prob. also some times the threads are worn or week and give. thats prob where the problem is- in the almn nuts. put them on just the triple alone and see if they go on smothly and are not lose.. sounds like you have alot of time in this one..

It's been awhile so don't remember about my 636, but there's 2 nuts below the tripple right?
If so then if you tighten the bottom one down, then put the tabed washer on then the 2nd nut down. If you can hold the bottom one in place while tightening the 2nd you are locking it in. If you do this your are pretty much "Jam Nutting" them and there's no way they'll move once you bend the tabs up. Just be careful not to cross -thread. I'd push it to the edge tho. That way they don't act as "one big nut" as mentioned earlier.
How much of your stem sticks up above the triple?
You could always put 3 nuts bellow the triple

Is there a difference between 92210A & 92210 ??? Maybe accidently swapped?
I honestly dont think the Hohey could be your problem......
Think about it like this, all the parts that are below the tripple are the ones that hold everything in place. the top tripple is only for stability of the forks, nothing to do with the stem being held in place.
The reason you have to adjust the hohey every time is because the nuts bellow that are backing off are pushing it up making it slide up the forks. IMO proof its not the top tripples fault. Its not meant to hold things down

i hate to say it. but i have turned some shitty bikes prety nice so i can say ive seen some junk. but on the neck there is an edge on the frame that holds the race in place. if the bike was ever hit hard in the front u should deff look.
if that edge is broken then the race (part between the frame and bearing) will wobble and cause it to lossen or you will likly always be able to tighten it more and more. deff might just wana jack it up and take it all off so you can look at everything if you wana do it right.
thats what was always wrong with that 929 (orlando vid off sand lake. bike w blue tank-and i had swamp ***) i put that 954 mtr in. it was hit hard but took the impact in the triple and broke off that edge i had it weilded and spent about 3 hours sanding it at your house. never did get it right so i always had that same problem.

Any one else that has one have problems with the lower triple stem constantly need to be tightened. My head bearings always seem to get loose. I have tried 3 different sets of bearings and lower triples but they All do the same thing so I know it is not the lower triple. Just wandering if any one else has noticed this?

Under the oem section is another good place to find diagrams to see how your bike goes back together... Instead of how you decided it was supposed to go lol
